New patients

How to register

1. Call us at 780-244-5231. We will book your first visit.
2. Fill out the registration form ahead of time to make check-in easy.Download the new patient registration form (PDF, 331 KB), print it and bring it with you. If you cannot print it, we will give you a copy at the clinic.
3. Read our patient policies before you come.
4. Bring your Alberta Health Care card and photo ID to your visit.

Your first visit

Your first visit is a chance to meet the doctor. It is not a medical assessment. No prescriptions, forms or diagnostic work will be completed during this visit unless agreed otherwise.

After the visit, the doctor will decide whether to accept you onto their panel.

• If you are accepted: you will be added as the doctor's patient (in CII/CPAR, Alberta's patient–doctor registry), appointments will be booked as needed, and you can contact the clinic for future care.
• If we cannot accept you at this time: we will let you know and help point you to other care options.

Patients from outside Alberta

If you have permanently moved to Alberta, your out-of-province health card is only valid for 3 months. If your out-of-province coverage is not valid, you will need to pay for your visit. See our fees.

What the form asks for

So you can have it ready, the registration form asks for:

• Your full name, date of birth and gender
• Your address and telephone number
• Your Alberta Health Care number (or out-of-province health number and address)
• An emergency contact: their name, relationship to you and phone number
• Your signature and the date

Your doctor may use secure software that helps record and write notes from your visit. This helps make sure your medical records are accurate.

For your privacy, please do not send health information by email. Bring your form in person.
